New Patient

Your first office visit will consist of a consultation explaining your condition, diagnosis and treatment alternatives. Sometimes, treatment can be initiated the same day as the consultation. However, a complex medical history or treatment plan will require a thorough evaluation and a second office visit to provide treatment on another day.

Please help us by providing the following information at the time of your consultation visit:
Your referral form and any radiographs if applicable.
All medications that you are presently taking.
Bring your dental or medical insurance information
IMPORTANT: A parent or guardian must accompany all patients under 18.
